Typhoid Fever in Nigeria: Signs, the Widal Test Problem and How to Prevent It
Many Nigerians say they have “malaria and typhoid” every few months. Real typhoid is a serious bacterial infection that needs the right antibiotic — but it is often wrongly diagnosed with the Widal test, and treated with the wrong drugs. Here is what you should know.
🦠 What causes typhoid
Typhoid is caused by the bacteria Salmonella Typhi. You get it from food or water contaminated with the faeces of an infected person — for example unsafe water, food handled with unwashed hands, or raw food washed with dirty water.
🌡️ Signs
Fever that rises day by day and lasts more than a few days
Headache, weakness and tiredness
Stomach pain, constipation or diarrhoea
Loss of appetite
In some people: a dry cough or rose-coloured spots on the chest
⚠️
Emergency signs
Severe stomach pain, vomiting blood, black stool, confusion or a very high fever can mean complications like a perforated (burst) intestine. Go to a hospital immediately.
🧪 The Widal test problem
The Widal test is cheap and common, but it is unreliable: it can be “positive” in people who had typhoid in the past, were vaccinated, or have other infections. That is why so many people are told they have typhoid when they don’t. A blood culture or stool culture is the more accurate test; ask your doctor about it, especially if fever keeps coming back.
💊 Treatment
Typhoid needs an antibiotic chosen by a doctor. Resistance to older antibiotics is common, so the right choice matters.
Take the full course, even when you feel better.
Drink plenty of clean fluids and eat light food.
Don’t buy antibiotics without a prescription or mix several “typhoid drugs” — it makes future infections harder to treat.
Boil water or use a trusted filter. Be careful with sachet water from unknown brands.

Frequently asked questions
Can malaria and typhoid happen together?
Yes, but it is less common than people think. Many “malaria and typhoid” diagnoses are based on an unreliable Widal test. Ask for proper tests.
How long does typhoid treatment take?
Usually 7–14 days of antibiotics, depending on the drug and how severe it is. Follow your doctor’s advice.
Is typhoid contagious?
It spreads through food or water contaminated by an infected person’s faeces, so hand washing and safe water stop it.
Can typhoid come back?
Yes, if treatment is incomplete, or if you are re-infected. Some people carry the bacteria without symptoms.
